Limited Durable Power of Attorney – Pre-Retirement – Applies to Employees in the State Employees Retirement System (SERS) Only
听Form CO-1049, the Limited Durable Power of Attorney (LDPOA), enables each member of the State Employees Retirement System (SERS) to authorize another person (an 鈥渁ttorney-in-fact鈥) to make retirement decisions and execute retirement documents in the member鈥檚 place, and on the member鈥檚 behalf. Having an LDPOA on file can be essential to protect the rights of members, their families, and other loved ones, if a member becomes incapacitated by illness or injury. Due to the significance of this document, the Retirement Services Division听strongly recommends听that you seek legal advice before signing this document. Every LDPOA is subject to review and approval by the Retirement Services Division. Completed forms should be returned directly to Human Resources.
